Effective November 4, 2026
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our duties. Anat Sapan MD is required by law to protect the privacy of your health information, give you this notice, follow its terms, and notify you if a breach compromises your unsecured health information.
How we may use and share your information without your written permission:
- Treatment: with pharmacies (including compounding pharmacies), laboratories, and other clinicians caring for you.
- Payment: to bill you and, if you request, to provide superbills for your own insurance submission.
- Health care operations: to run the practice, improve care, and work with vendors (such as our electronic health record) who sign business associate agreements.
- As required or permitted by law: public health reporting, abuse or neglect reporting, health oversight, court orders, law enforcement in limited cases, to avert a serious threat to health or safety, workers' compensation, and to coroners or for organ donation as the law allows.
Uses that require your written authorization: marketing, any sale of your information, and most sharing of psychotherapy notes. You may revoke an authorization in writing at any time.
Your rights. You have the right to:
- Get an electronic or paper copy of your records, usually within 30 days.
- Ask us to correct information you believe is wrong.
- Ask us to communicate with you confidentially, for example at a different address or phone.
- Ask us to limit what we use or share. We must agree if you paid in full out of pocket and ask us not to share with your health plan.
- Get a list of certain disclosures we made in the past six years.
- Get a paper copy of this notice at any time.
- Choose someone, such as a medical power of attorney, to act for you.
State law. Where California, Florida, Illinois or New York law gives your information more protection than HIPAA, such as for HIV, mental health, or genetic test results, we follow the stricter law.
Complaints. If you believe your privacy rights were violated, contact our Privacy Officer, Anat Sapan, MD, at info@doctoranat.com or 510-929-4104. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints. We will not retaliate against you for filing a complaint.
Changes. We may change this notice. The new notice will apply to all information we hold and will be posted on this page.
